Form Directions

All dimensions are in inches. For divisions of inches, use decimals.
(For Example: For a width of 37-1/4, enter 37.25).
For double-hung windows, the height measurement is not required. For all other windows, the height is required.
* Please note that there cannot be more than a 4-inch opening anywhere in the guarded area.
Be sure to choose the correct window type. Choosing the wrong type can result in improperly guarded windows. For more information on window types and how to measure your windows, refer to the measuring instructions above.
